Housing Case Manager

The Mental Health Association of San Mateo County (MHA), a nonprofit organization, enriches quality of life, restores dignity, and reduces homelessness, particularly for those affected by mental illness and HIV/AIDS. MHA strengthens hope by providing shelter, support and opportunities for socialization.

The Mental Health Association of San Mateo County is recruiting for an experienced, full-time Case Manager to help homeless and formerly homeless adults living with mental illness to secure and maintain long term and permanent housing.

Working closely with other community agencies, the Housing Case Manager will provide support to the 15 residents of Spring Street Shelter, an emergency shelter with a typical stay of 45 days. The focus of the shelter case manager's job is work with each resident and/or with their outside treatment providers, to develop and implement a plan to transition the resident into stable housing. The Housing Case Manager will work in a supportive manner to determine each resident's housing goal and the tasks necessary to reach that goal. The Housing Case Manager will actively develop community resources and cultivate connections that will help realize the resident's housing goal. On occasion the Housing Case Manager will provide backup to the Shelter Site Manager.

The Housing Case Manager will also provide intensive case management support to seven residents living in transitional housing, all of whom are former Shelter residents. Upon moving into the Transitional Housing units, resident will become tenants and program participants and may live there for up to eighteen months. The Housing Case Manager will develop a comprehensive case plan in collaboration with each resident. The expectation is that each resident participates in at least 20 hours of activity every day directed to achieve the goals of the case plan. The Housing Case Manager provides ongoing support for clients while continually assessing their need for increased mental health services, health and other services and providing crisis intervention when needed. The Housing Case Manager assists and supports residents in obtaining needed mental health, medical, social service, and entitlement program services. The Housing Case Manager supports individuals to learn and apply independent living skills, assists with budgeting on a fixed income, and helps clients maintain good mental and physical health.

The Housing Case Manager will also provide six months of follow-up assistance to each graduate of transitional housing to ensure the client resides stably in housing. The Housing Case Manager will enter client information into client databases and prepare annual reports.

We're looking for a go-getter who will do everything possible to house our clients. The ideal candidate will be familiar with local resources or have a history of actively engaging local nonprofits and housing resources. Requirements for the position include at least two years of case management experience working with people with serious mental illness, a B.A. in psychology, social work or related field, initiative, dependability, good computer skills and flexibility. All applicants must pass a criminal justice background check upon hire and must have a valid CA driver's license with a good driving record, a reliable car, and current automobile insurance. MHA reimburses for mileage.

This is a full-time position that will require working Tuesday through Saturday and some evenings . Compensation will be based upon candidate's level of skills and experience. The Mental Health Association of San Mateo County is an equal opportunity employer.
