SAYAT
The Support and Advocacy Program for Young Adults in Transition (SAYAT) offers intensive case management and support services to young adults who require assistance to live independently in the community. The purpose of the program is to provide training and assistance to disenfranchised, disabled young adults to develop the practical skills and goals necessary for independent and successful community living.
Location: San Mateo County
Services
Case managers work with each SAYAT client to:
- Find housing.
- Return to school.
- Obtain vocational training.
- Obtain a job.
Eligibility
Participants must:
- Be between 18 and 23 years of age.
- Be considered homeless or at risk of homelessness (as defined by HUD).
- Reside in San Mateo County.
Application Information
In order to apply, a referral form must be completed and submitted to the MHA. Referrals can be downloaded and submitted to the MHA by fax at (650) 368-9017 (Note: referrals are not accepted by email). Case managers will contact the referring individual to obtain additional information regarding prospective client.
